You can start <a href=\"\/knowledge-base\/the-library\/#edit-a-questionnaire-in-the-library\">adjusting this template<\/a> to your specific study. You can use all the different <a href=\"\/article-categories\/question-types\/\">question types<\/a> in your informed consent.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Create new informed consent<\/h4>\n\n\n\n<p>You can also start creating a new informed consent from scratch. Creating an informed consent is quite similar to creating a questionnaire.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button is-style-outline is-style-outline--1\"><a class=\"wp-block-button__link has-text-color wp-element-button\" href=\"https:\/\/manual.m-path.io\/knowledge-base\/create-a-questionnaire\/\" style=\"color:#03a9f3\">Learn more about creating a questionnaire<\/a><\/div>\n<\/div>\n\n\n\n<p>We will give a few examples on how you can create an informed consent.<\/p>\n\n\n\n<p><strong>Example 1: Informed consent as a whole<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Create a new interaction (questionnaire) and name it <em>Informed consent<\/em>.<\/li>\n\n\n\n<li>Add a <a href=\"\/knowledge-base\/media-items\/#text\">Media item (text)<\/a>. Enter an introductory text. Check the <em>Formatted text<\/em> box. Check the <em>Customize &#8216;Next&#8217; button<\/em> box and enter the text you want to display on the button (e.g. <em>Continue<\/em>).<\/li>\n\n\n\n<li>Add a <a href=\"\/knowledge-base\/container\/\">Container<\/a>. Check the <em>Display on one page<\/em> box. Check the <em>Has next button<\/em> box. Check the <em>Customize &#8216;Next&#8217; button<\/em> box and enter the text you want to display on the button (e.g. <em>Continue<\/em>).<\/li>\n\n\n\n<li>Add Media items (text) to the Container (by clicking on the upside down arrow next to <em>Container<\/em>). Every item can contain a part of your informed consent. Check the <em>Formatted text<\/em> box for every item.<\/li>\n\n\n\n<li>Add a <a href=\"\/knowledge-base\/yes-no-question\/\">Yes\/No item<\/a> (outside of the container). Enter the question or statement you want to display. If you would like to make use of the <em><a href=\"\/knowledge-base\/informed-consent\/#start-study-after-consent-is-given-in-intake\">Start study after consent is given in intake<\/a><\/em>, make sure to start the label of this item with <em>&#8216;consent&#8217;<\/em>. Check the <em>Formatted text<\/em> box for every item.<\/li>\n\n\n\n<li>Add a <a href=\"\/knowledge-base\/yes-no-question\/\">Yes\/No item<\/a> (outside of the container). Enter the question or statement you want to display. If you would like to make use of the <em>Start study after consent is given in intake<\/em>, make sure to start the label of this item with <em>&#8216;consent&#8217;<\/em>. This way, the informed consent will stop once the participant has answered &#8216;no&#8217; to one of the Yes\/No (&#8216;consent&#8217;) questions.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button is-style-outline is-style-outline--2\"><a class=\"wp-block-button__link has-text-color wp-element-button\" href=\"\/knowledge-base\/conditional-question\/\" style=\"color:#03a9f3\">Learn more about conditional questions<\/a><\/div>\n<\/div>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Schedule informed consent<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">Schedule informed consent manually<\/h4>\n\n\n\n<p>If you are not working with a <a href=\"\/knowledge-base\/protocols\/\">protocol<\/a> or intake interaction, you can also schedule your informed consent manually. Scheduling the informed consent manually is similar to scheduling a questionnaire manually.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button is-style-outline is-style-outline--3\"><a class=\"wp-block-button__link has-text-color wp-element-button\" href=\"\/knowledge-base\/scheduling-interactions\/#manually-fill-in-schedule\" style=\"color:#03a9f3\">Learn more about scheduling a questionnaire manually<\/a><\/div>\n<\/div>\n\n\n\n<h4 class=\"wp-block-heading\">Schedule informed consent in protocol<\/h4>\n\n\n\n<p class=\"wp-block-ht-blocks-messages wp-block-hb-message wp-block-hb-message--withicon is-style-info\">Creating and using protocols are <strong><a href=\"\/knowledge-base\/subscriptions-and-billing\/\">premium<\/a> <\/strong>functions.<\/p>\n\n\n\n<p>If you are not working with an intake interaction, you can schedule your informed consent to be the first interaction in your protocol.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button is-style-outline is-style-outline--4\"><a class=\"wp-block-button__link has-text-color wp-element-button\" href=\"\/knowledge-base\/protocols\/\" style=\"color:#03a9f3\">Learn more about creating and using protocols<\/a><\/div>\n<\/div>\n\n\n\n<h4 class=\"wp-block-heading\">Set informed consent as intake interaction<\/h4>\n\n\n\n<p>You can set the informed consent as the intake interaction by going to the <a href=\"\/knowledge-base\/auto-enroll-settings\/\" data-type=\"ht_kb\" data-id=\"11091\">auto-enroll settings<\/a>, checking the <em>Simple intake<\/em>&nbsp;box and selecting your informed consent from the&nbsp;<a href=\"\/knowledge-base\/the-library\/\">library<\/a>.<\/p>\n\n\n\n<h5 class=\"wp-block-heading has-text-color\" style=\"color:#000000\"><strong>Start study after consent is given in intake<\/strong><\/h5>\n\n\n\n<p class=\"wp-block-ht-blocks-messages wp-block-hb-message wp-block-hb-message--withicon is-style-info\">The <em>Start baseline schedule after consent is given in intake<\/em> option is a <strong><a href=\"\/knowledge-base\/subscriptions-and-billing\/\">premium<\/a> <\/strong>function.<\/p>\n\n\n\n<p>When the option <em>Start baseline schedule after consent is given in intake<\/em> is checked in the <a href=\"\/knowledge-base\/auto-enroll-settings\/\" data-type=\"ht_kb\" data-id=\"11091\">auto-enroll settings<\/a>, the protocol will only be loaded and started after the intake questionnaire has been filled in by the participant and questions with a label that starts with &#8216;consent&#8217; have been answered with yes. This can be used to make sure that participants who don&#8217;t give consent won&#8217;t receive further questionnaires. For example, a Yes\/No question &#8216;Do you give consent to use your answers in researcher&#8217; with label &#8216;consent_question_1&#8217; can be added to the intake questionnaire.
